Soccer is a contact sport and, as such, injuries do happen.
Whilst Turf City Football Club will take every precaution to ensure injuries do not occur during their training sessions, Turf City Football Club, their coaches, staff and players will not be held responsible or liable for any permanent injuries or losses of property to your child that may occur arising from any cause whatsoever during their time at a coaching session.
Parents and players are advised to get themselves covered with your own personal insurance in relation to any sports-related injuries.
Should Turf City Football Club be unable to contact the child's parents, I hereby give my permission to Turf City Football Club to seek medical assistance for my child. I will bear full financial responsibility for any costs incurred.
Turf City Football Club reserves the right to use any photographs and / or videos taken at Turf City Football Club training sessions and matches in and outside of Singapore for publicity purposes.
The person entering this form MUST be the child's parent or legal guardian.
